Hello, In order for us to recertify you, you MUST have a copy of your expired certificate or wallet card, or any other document as part of a proof that you WERE PREVIOUSLY CERTIFIED, in your possession.

If you have such document with you right now, you have two options listed below:

Upload a copy of your expired certificate, using the browse button below: Select jpg/tiff/pdf/gif image file of your expired certificate to upload: Note: Maximum File size 9 MB, you may zip your files and upload. You can also include your picture, if you want a photo id.	If you are faxing your expired certificate, please fax it to 1-888-666-2841 & then call us.
